Authorization and Consent to Release Information
A personal training client agreement form should be completed before starting any type of exercise program. This document ensures that both the client and the personal trainer are aware of their rights and responsibilities. The personal trainer should also have a copy on file in order to meet any legal requirements.
The first section of the form is titled “Authorization.” The personal trainer must obtain written authorization from the client to release any information related to their fitness program. This includes medical information, weight, and fitness goals.
The second section of the form is titled “Consent.” The consent clause states that the client has agreed to allow the personal trainer access to their records, photographs, and other materials related to their fitness program. The consent must be verbal and in writing for it to be valid.
If either party does not agree to any parts of the agreement, they should not sign it. The personal trainer may still begin working with the client, but they will not be able to release any information until it has been revised and approved.
Indemnification
As a personal training client, you agree to indemnify and hold harmless the personal training instructor and the studio against any and all claims, losses, damages, costs, and expenses (including reasonable attorneys’ fees) arising from or in any way related to any injuries that you may cause to yourself or another person during your visit to the studio.
Waiver of Legal Rights
Many people are hesitant to sign a waiver of legal rights before beginning personal training, but doing so can be very beneficial. By signing the waiver, you are acknowledging that you understand and agree to the following terms:
-You relinquish any right to sue or hold the trainer liable for any injuries that may occur during your session.
-You release the trainer from all liability arising from any comments or suggestions made during your session.
-You waive any right to privacy regarding your session or any information disclosed in connection with it.
-You consent to appear in photographs or videos taken during your session, whether or not you are aware of it at the time.
Miscellaneous Provisions
A personal training client agreement form will outline important details about your working relationship with the personal trainer. This document should include information about your payment schedule, start and end dates of services, and any other conditions or stipulations associated with your contract. It is also important to include a waiver of liability clause, in case something goes wrong while you are working out with the trainer.
